Differences in patterns of survival in metastatic adenoid cystic carcinoma of the head and neck.

چکیده
BACKGROUND We examined the assumption in conventional teaching about metastatic adenoid cystic carcinoma (ACC) being an indolent type of disease. METHODS A single center analysis of 105 cases of ACC was performed. Radiographs were reviewed and tumor response to chemotherapy was measured. Distant disease-free survival (DDFS) and time to death since distant metastases diagnosis were analyzed. RESULTS Forty-two percent of the patients were diagnosed with distant metastases. DDFS showed significant negative associations with advanced T classification, N+ classification, solid type tumor, and positive surgical margins. Distant metastases (91%) developed in the first 5 years after presentation. Median distant metastatic survival was 13.8 months. The most frequent organ sited was the lung. Solid type ACC showed a preponderance for multiorgan metastases (17/28; 61%). Distant metastases seemed not to occur in case of clear surgical margins. Solid type ACC had a significant poorer survival after development of distant metastases. CONCLUSION Metastatic ACC is not always an indolent disease. © 2016 Wiley Periodicals, Inc. Primary cutaneous adenoid cystic carcinoma of shin
Primary cutaneous adenoid cystic carcinoma (PCACC) is a rare variant of adenoid cystic carcinoma (ACC) which originates from the skin adnexa, most probably eccrine and apocrine glands. This tumor has a head and neck predilection and also occurs in the chest, abdomen, back, and perineum. We hereby present a 63-year-old man with PCACC located on the shin. No previous similar case has been reporte...
متن کاملNasopharyngeal Carcinoma with Cystic Cervical Metastasis Masquerading as Branchial Cleft Cyst: A Potential Pitfall in Diagnosis and Management
Introduction:Most metastatic lymph nodes from head and neck malignancy are solid. Cystic nodes are found in 33% - 61% of carcinomas arise from Waldeyer’s ring, of which only 1.8% - 8% originate are from the nasopharynx. Some cystic cervical metastases were initially presumed to be branchial cleft cyst. This case report aims to highlight the unusual presentation of cystic cervical metastasis sec...

متن کاملRelations among expression of CXCR4, histological patterns, and metastatic potential in adenoid cystic carcinoma of the head and neck.
Adenoid cystic carcinoma (ACC) may acquire a chemokine-mediated mechanism during the process of metastasis. To investigate the involvement of chemokines in metastasis from ACC, expression of CXCR4 in surgical specimens of ACC and two tumor lines transplantable to nude mice was examined immunohistochemically. In addition, the expression levels of CXCR4 protein and mRNA were examined by Western b...
